1. Will my insurance cover this visit?

Each insurance plan is different. It is best to contact your insurance carrier to get an accurate explanation of your benefits.

2. How often will my insurance cover an annual exam or physical?

Each insurance plan is different. It is best to contact your insurance carrier to get an accurate explanation of your benefits. Many insurance companies will cover a physical exam every year, providing it has be at least one year and one day since the previous one. It is always best to check with your insurance.

3. How do I get a prescription refilled?

Call your pharmacy for all refills. They will fax a request to our office. This can take up to 2 business days, so it is best to plan ahead.

4. Are your doctors accepting new patients?

Many of our doctors are accepting new patients. Please call our clinics directly and our receptionists will be happy to provide you with the names of those physicians.

All new patient forms are available on this website. We suggest that you bring the completed forms with you at the time of your visit. If you prefer to complete them at the clinic, please plan to arrive 15 minutes ahead of time.

5. How can I reach a Cascade doctor after hours?

For after hours care, call the clinic phone number and a physician will call you back.

6. Do you have parking at all of your locations?

Yes, all of our clinics have convenient, free parking. Our Bridgetown clinic, at Emanuel hospital, offers valet parking.

7. Are you accepting new Medicare or Oregon Health Plan patients?

At this time we are not accepting any new patients with traditional Medicare or Oregon Health Plan insurance. However, we do accept some Medicare plans. Please refer to the Insurance Plans page for a listing.

8. I am having a lab test done. Do I need to fast?

If you are having your cholesterol or blood sugar levels checked, you will need to fast for 12 hours prior to the test. You may have water, black coffee or tea. For other specific tests, please call the clinic for instructions.

9. I have a question on my bill. Where do I call?

Our billing office will be happy to assist you with any billing questions that you might have. They can be reached at (503)242-9814.

10. How do I get my medical information sent to/from another physician's office?

A records release form needs to be completed. These forms are available at our office or can be printed from this web site. Make sure that you complete the name, address and fax number of the doctor or facility you are requesting records to or from. This process can take up to one month to complete.

11. It has been a few years since I have been seen in your clinic. Am I still considered a patient there?

If it has been more than three years, you will be considered a new patient and will need to follow the same guidelines as all new patients. If it has been more than one year since you have been seen, please arrive 15 minutes early for your appointment so that we can update all of your information. This is necessary even if there have been no changes.

13. What hospitals are you affiliated with?

The doctors at Cascade Physicians admit to Legacy Good Samaritan, Emanuel and Meridian Park hospitals. All of the physicians at the Bridgetown Clinic also admit to Portland Providence.
